Patients with insurance...
We're sure you're aware that billing and collecting from your insurance company has become a nightmare. For this reason we are asking for your help.
We will gladly file your insurance claims for you. Insurance reimbursement will be sent directly from your insurance company to us. Please be aware that some, and perhaps all, of the services provided may be non-covered services and not considered reasonable and necessary under your insurance. You are responsible for payment regardless of your insurance company's arbitrary determination of usual and customary rates.
For your convenience several payment options are available: We accept Cash, Checks under $40, MasterCard or Visa
Insurance plans with a co-pay will be collected at every visit. Some of these plans may also have a deductible and coinsurance as well that may apply to surgery / procedures done within the office.
Your claim will be submitted to your insurance and you will be billed any remaining balance your insurance deems as uncovered charges.
Insurance plans with a deductible will require the patient to meet that deductible amount mandated by your insurance to be paid before your coinsurance percentage can be collected. Therefore, at the beginning of the calendar year or your effective date with your insurance, you will be expected to pay the entire portion of your visit charge at the time of your service rendered. After your deductible has been met then you will be responsible for your coinsurance percentage amount every visit.
Self pay Patients without Insurance...
We will gladly see patients without insurance. Our practice is committed to providing the best treatment for our patient and we charge what is usual and customary for our area. The cost of your visit is determined by the complexity of the visit as determined by, the length of time taken to consult and examine you during your visit, procedures done in the office the decision making process and so on. Therefore, we are unable to give you a set amount of how much a visit will cost you until after your visit has been completed.
Once services are rendered, you will be expected to pay the entire portion of your visit charge(s).
