First Visit Procedures
Please bring the following items with you to your appointment:
- Picture ID (i.e. Driver license, Military ID, School ID & etc.).
- Insurance Card(s).
- Co-payment (Visa, MasterCard, checks or cash).
- List of medications with dosage or the actual bottles that you are currently on.
- Children visits: Please bring immunizations records.
- Hospital follow-up visit: Please bring any records from the hospital.
- Bring all filled out paperwork from our website (see below).
